快速诊断寄生虫感染的免疫层析检验

鼠肺虫广州管圆线虫是动物体内的一种食物传播寄生虫,它致人得嗜酸性脑膜炎。只有在病人脑脊液中发现线虫才能确认疑似感染广州管圆线虫的病例。
针对人感染广州管圆线虫的血清诊断现在仍有问题,因为没有经过确证的市售检验产品。大多数实验室使用自己制备的检验产品,如酶联免疫吸附测定(ELISA)或免疫印迹。
泰国曼谷玛希隆大学(www.mahidol.ac.th)的科学家与其他科学家合作,评估了97份血清样本:19份样本来自临床确诊的患者,用免疫印迹法可检测到他们体内广州管圆线虫特异的抗体;43份样本来自其它寄生虫病患者,如颚口线虫病、弓首线虫病、旋毛虫病、钩虫感染、丝虫病、囊尾蚴病、肺吸虫病、后睾吸虫病和疟疾;35份样本来自正常的健康受试者。
用纯化的31-kDa糖蛋白作为诊断用的抗原,用金标记的抗人免疫球蛋白G作为检测器试剂,组装成这款检验器件。科研小组评估了快速侧流免疫层析测定——AcQuickDx检验检测人血清中抗广州管圆线虫抗体的效果。15分钟内T线和C线上出现红带就说明是阳性反应,而T线没有红带、C线出现红带说明是阴性反应。
科研小组发现,对于经血清检验确认的管圆线虫病例,AcQuickDx检验检测抗广州管圆线虫特异抗体的灵敏度、特异性、阳性预测值与阴性预测值分别为100%、98.72%、95%和100%。1/4的钩虫感染病例观察到AcQuickDx阳性结果。其它寄生虫病病例和健康受试者没观察到AcQuickDx阳性结果。
该研究的论文发表于2018年8月号的《国际传染病杂志》(International Journal of Infectious Diseases)。作者总结说,AcQuickDx检验速度快,灵敏度与特异性高,容易执行,无需其它设备或辅助材料。生成的结果用目测可解读,且室温下的保存期长。因此,它可作为一项备选检验,支持管圆线虫的临床诊断,既可在常规实验室使用,也可用于边远地区。
