耐药细菌正在全世界蔓延,必须快速进行抗生素敏感性试验,确定最小抑菌浓度(MIC),以便及时选择恰当的抗生素疗法。
抗生物的使用日渐泛滥,且经常不必要,导致病原体产生耐药性,本应很容易治愈的感染也许会致命。一种新型的快速检验将得知哪些现有的抗生素仍有效,更快的诊断可实现个性化疗法,挽救生命。
德国耶拿大学医院的科学家及其同事开发了一种简单快速的拉曼谱检查,以鉴定对抗生素的敏感性,短短两小时就可完成总体分析,确定MIC,比今天诊断所用的成熟的表现型方法节省大量时间。
样品制备也比现行的成熟检验更快、更容易。介电泳芯片可自动收集微米见方的细菌,直接用细菌悬浮液测得高质量拉曼谱。用13份大肠杆菌临床分离物确证新型拉曼谱MIC检验,这些分离物来自血流感染患者,表现出对环丙沙星不同程度的耐受性。
微拉曼谱能够检测与环丙沙星相互作用90分钟后大肠杆菌的变化。进行主分量分析并简单计算1458和1485 cm–1处拉曼标志带的比率,结果显示出清晰的浓度依赖效应。新型拉曼谱方法确定的MIC值与微量肉汤稀释法、Vitek-2和法国生物梅里埃公司的E-test等参考方法得到的MIC值吻合得很好,可用于区分是敏感、中等还是耐受。
该研究的论文发表于2018年2月号的《分析化学》(Analytical Chemistry)杂志。资深作者、物理化学教授Ute Neugebauer博士说:“我们联用基于光的分析方法与微流控样本处理。我们的芯片实验室系统是一个微型实验室,有了它,我们就能明确鉴定菌株及其耐药性,需时不到三小时。”
