预后不佳的早期癌症CTC指标

检测肺肿瘤部位附近肺静脉血液而非外周静脉血,癌症研究人员发现,疾病早期血液中有循环肿瘤细胞(CTC),是患者预后不佳的一种指标。
由于早期阶段外周血中CTC数量有限,University of Michigan(Ann Arbor,USA)的研究者检验了肿瘤切除术期间获得的肺静脉血中CTC的水平。手术期间抽取肺癌患者的肺静脉(PV)和外周静脉(Pe)血液标本,使用微流设备评估CTC负荷。
分析36名患者的108份血液样本,PV的CTC数量明显高于术前Pe和术中Pe血液。50%的患者可见CTC团和大量CTC,常发现PV中有更大的CTC团。长期监测显示,术前Pe血液中存在CTC团预示有预后不佳的趋势。
基因肿瘤学分析揭示,CTC团中富集细胞迁移和免疫相关通路,提示循环中有CTC团的患者有生存优势。CTC团显示出治疗耐受性特征,表明这些细胞具有侵袭性。因此,从早期肺癌患者中分离出CTC是预后不佳的先兆,并能分析以确定预测复发的生物标志物。
“肿瘤不断脱落细胞,即使肿瘤较小时也是如此,这是我们了解到的东西,”资深作者,University of Michigan的化学工程教授,Sunitha Nagrath博士说。“虽然我们定义肿瘤为早期,但它们已经在体内传播细胞了。即使已切除肿瘤,仍有数千数万个细胞留在患者体内了。如果您知道已离开诊所的患者会因为这些残留的细胞在不足一年内复发,那为什么不现在就进行治疗呢?”
