Существуют ощутимые потребности в точном измерении низких концентраций эстрадиола (E2), составляющих менее 20 пг/мл, у женщин в постменопаузальный период, мужчин, пациентов детского возраста и пациентов, проходящих лечение рака молочной железы.
Автоматизированные методы иммунологического анализа являются наиболее широко используемыми методами для измерений E2 в клинических лабораториях; однако им недостает точности и точности измерения при низких физиологических концентрациях E2. На основе микрожидкостной хроматографии – тандемной масс-спектрометрии (micro LC-MS/MS) был разработан высокочувствительный метод без дериватизации, чтобы достоверно измерять концентрации E2 ниже 5 пг/мл с использованием небольшого объема пробы.
Ученые из Чикагского университета (штат Иллинойс, США) использовали образцы для сравнения методов из оставшихся образцов, доставленных из лаборатории клинической химии. В общей сложности 290 мкл образца было смешано с внутренним стандартом, E2-d4, и извлечено со смесью гексана/этилацетата (процентное соотношение объёмов 90/10). После извлечения образец был отделен с помощью системы Eksigent Ekspert micro LC 200 с расходом жидкости, равным 35 мкл/мин. при полном времени пробега, равном 3.5 минут, и обнаружен с помощью масс-спектрометра QTRAP 6500 (Sciex, Фрамингем, штат Массачусетс, США) в режиме отрицательной ионизации, используя переходы: 271/145 (квантификатор) и 271/143 (квалификатор). В этом методе было крайне важно использовать колонки для проведения жидкостной хроматографии высокого давления, имеющие стабильность при pH >10.

Метод микрожидкостной хроматографии и тандемной масс-спектрометрии показал хорошую корреляцию с радиоиммунологическим анализом и невысокую корреляцию с автоматизированным методом иммунологического анализа E2 Roche Cobas. Валидационное исследование показало широкие линейные диапазоны от 3.0 до 820 пг/мл. Итоговая точность измерений была ниже 15% на всех уровнях контроля качества, и нижняя граница определяемых концентраций составляла 3.0 пг/мл.
