房颤(AF)属一种心律异常,会增加脑卒中与心力衰竭的风险,但常因缺乏症状而诊断不出来。房颤甚至会增加痴呆的风险。
 
研究发现,房颤患者血液循环中的四段短RNA (miRNA)表达水平升高。这些miRNA有望用作预测房颤发作的生物标志物。miRNA控制转录后的基因表达,它在血流中十分稳定,以前的研究已表明它可能是某些心血管疾病的标志物。
 
日本东京医科与口腔科大学的一组科学家招募了60名房颤患者和55名对照者。房颤组有30名阵发性房颤(PAF)患者和30名慢性房颤(CAF)患者。对照组有30名无房颤相关症状的年龄相当的对照者(AC)和25名健康的年轻对照者(YC)。从所有人中选出15名年龄相当的受试者,全面分析miRNA,对其余100名受试者做个体miRNA表达分析。
 
从外周静脉中采集血样,以1,600倍重力加速度离心10分钟,获得血清,并在−80 ℃储藏,直到分离出RNA。用美国加利福尼亚州圣克拉拉市赛默飞世尔科技公司的mirVana PARIS试剂盒提取总RNA。通过定量逆转录酶聚合酶链反应(RT-PCR)量化个人血清中miRNA的表达。分离得到的总RNA样本(每份2微升)经过逆转录成为cDNA,再用赛默飞世尔科技公司的TaqMan miR探针量化。在赛默飞世尔科技公司的Step One实时PCR系统上执行PCR反应。研究者还用了一只小鼠模型。
 
对个体的评估发现,房颤患者体内四种miRNA水平显著升高:miR-99a-5p、miR-192-5p、miR-214-3p和miR-342-5p。对诊断能力的统计分析显示,miR-214-3p和miR-342-5p作为预测房颤的生物标志物的准确度最高,但是对全部四种miRNA的组合分析只略微提高了准确度。
 
该研究的论文发表于2018年2月5日的《循环杂志》(Circulation Journal)。通信作者Tetsuo Sasano博士说:“一部分间歇性房颤患者以及另外一些慢性房颤患者体内仍是那两个miRNA的表达水平升高。相较同龄的健康对照者与健康的年轻对照者都升高,说明这些miRNA也许能预测房颤,无论人的年龄如何。”
