检测肺肿瘤部位附近肺静脉血液而非外周静脉血,癌症研究人员发现,疾病早期血液中有循环肿瘤细胞(CTC),是患者预后不佳的一种指标。
由于早期阶段外周血中CTC数量有限,University of Michigan(Ann Arbor,USA)的研究者检验了肿瘤切除术期间获得的肺静脉血中CTC的水平。手术期间抽取肺癌患者的肺静脉(PV)和外周静脉(Pe)血液标本,使用微流设备评估CTC负荷。
分析36名患者的108份血液样本,PV的CTC数量明显高于术前Pe和术中Pe血液。50%的患者可见CTC团和大量CTC,常发现PV中有更大的CTC团。长期监测显示,术前Pe血液中存在CTC团预示有预后不佳的趋势。
基因肿瘤学分析揭示,CTC团中富集细胞迁移和免疫相关通路,提示循环中有CTC团的患者有生存优势。CTC团显示出治疗耐受性特征,表明这些细胞具有侵袭性。因此,从早期肺癌患者中分离出CTC是预后不佳的先兆,并能分析以确定预测复发的生物标志物。
